Responsibilities
- Provide direct and indirect skilled nursing care for adult and geriatric residents under the supervision of a Registered Nurse.
- Collect data on residents’ physical and psychosocial status, document and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the development of an evidence-based, age-appropriate care plan reflecting nursing diagnoses and interventions; involve residents, families, and the healthcare team to achieve desired outcomes.
- Recommend revisions to the plan of care after evaluating resident outcomes; initiate referrals to address all aspects of resident care as needed.
- Collaborate with residents/families and interdisciplinary team to ensure care is safe, effective, and culturally appropriate; promote a healthy and secure care environment.
- Maintain current knowledge and clinical competency; participate in mentoring or orientation of other staff; provide leadership under the direction of the RN.
- Adhere to established policies and procedures; protect staff and resident safety; comply with safety standards for potential exposure to bloodborne pathogens.
Requirements
- Active New York LPN license
- BLS certification through American Heart Association or RQI
- Minimum 1 year of LPN experience in a long-term care facility
- Completion of an approved LPN program (education)
- Ability to provide care to adult and geriatric residents within a long-term care setting
- Must be able to start full-shift orientation on a Tuesday or Thursday
