Responsibilities
- Assess patients’ functional abilities and contribute to evaluation and reassessment of therapy needs.
- Provide direct therapeutic treatments and implement therapy plans within the scope of practice.
- Apply appropriate modalities and activities to promote functional gains and independence.
- Document evaluations, treatment goals, progress notes, and communicate with the interdisciplinary team.
- Educate patients and family members on treatment rationale, home exercise programs, safety, and equipment use.
- Collaborate with occupational therapy and other health professionals to coordinate care and adjust plans as needed.
Requirements
- License Requirements: Must be a licensed Occupational Therapist in Vermont and New Hampshire or hold an unencumbered compact license.
- Certification Requirements: Must have passed National Registration Exam.
- Education Requirements: BS in Occupational Therapy or postgraduate Occupational Therapy Certification program.
- Experience Requirements: Experience in a home health setting preferred.
- Additional: Ability to work effectively in a home health/hospice care environment.
