Responsibilities
- Provide direct nursing care to residents in a long-term care setting per established care plans and physician orders.
- Administer prescribed medications and treatments, document responses, and report changes in patient condition to the supervising clinician.
- Conduct regular assessments, monitor vital signs, and update care plans accordingly.
- Collaborate with an interdisciplinary team to coordinate resident care, including documentation and communications with physicians, therapies, and families.
- Maintain safety and infection control practices, manage routine procedures within scope of practice, and ensure accurate charting and record-keeping.
Requirements
- Active Licensed Practical Nurse license (state-specific licensure requirements to be confirmed per job notes)
- Certifications and education requirements as indicated in the Job Notes
- Experience in a long-term care setting as applicable to the job notes
- Ability to work in a local assignment and comply with the stated shift schedule
- Willingness to work a 3x12-hour shift pattern and meet guaranteed hours
