- Collaborate with nursing, therapy aides, and other members of the interdisciplinary care team.
- Educate residents and families on home programs, safety, and prevention strategies.
- Ensure timely, accurate documentation and adherence to facility policies and regulatory requirements.
Requirements
- Current, unrestricted Physical Therapist license (verification and specifics to be confirmed)
- Experience in a skilled nursing facility or long-term care setting preferred
