Responsibilities
- Conduct initial and ongoing assessments of residents’ functional status and PT needs.
- Develop and implement individualized treatment plans focusing on mobility, strength, balance, gait, and functional independence.
- Provide therapeutic interventions including therapeutic exercise, gait training, ROM, and balance training.
- Document progress and adjust plans based on patient response and interdisciplinary team input.
- Communicate and coordinate with physicians, nursing staff, therapy assistants, family, and care teams to support discharge planning and goals.
- Ensure safety, infection control, and equipment maintenance, while promoting patient education and self-management.
Requirements
- Active Physical Therapist license or eligibility for licensure in a manner appropriate to the assignment.
- Experience in geriatric or skilled nursing settings preferred.
- Strong assessment, treatment planning, and documentation skills.
- Ability to work in a skilled nursing facility environment and collaborate with an interdisciplinary team.
- Availability for a travel assignment with a consistent, 5-day workweek and 8.5-hour shifts (08:00–16:30).
