Physical Therapist, General — Tillamook, OR. Travel assignment starting 10/05/2026 for 24 weeks, with guaranteed 40 hours per week on a 4x10-hour shift. Weekly pay ranges from $2,596 to $2,752. Responsibilities
- Perform PT evaluations and establish individualized treatment plans to address physical function and mobility goals
- Plan and execute therapy interventions using evidence-based techniques; adjust as needed based on patient progress
- Co-lead and participate in interdisciplinary patient groups and care planning
- Develop and communicate discharge plans; ensure continuity of care with appropriate stakeholders
- Document assessments, plans, progress notes, and outcomes in a timely, accurate manner
- Supervise assigned assistants and collaborate with other care team members to optimize patient outcomes
Requirements
- Education: Doctor of Physical Therapy (DPT) required; only if licensure obtained on or after January 2016, or foreign equivalent
- Licensure: Physical Therapy licensure in the state of practice required
- Certifications: CPR/BLS certification from an approved vendor required
- Experience: Two years of Physical Therapy experience preferred; at least two years’ recent field experience noted in job requirements
- Additional: Current nursing license per state of assignment; ability to work in a travel assignment in a rural setting
