This travel PT position supports a short-term rehabilitation and skilled nursing setting in Napa, CA. The assignment is 13 weeks with guaranteed 40 hours per week and a flexible schedule. A float between two nearby facilities within walking distance may be required to meet patient and facility needs.

Responsibilities
- Perform initial evaluations and develop individualized treatment plans for residents in a SNF setting.
- Administer and supervise rehabilitation therapies to improve mobility, strength, balance, endurance, and functional independence.
- Monitor patient progress, document outcomes, and adjust treatment plans accordingly.
- Collaborate with an interdisciplinary team to coordinate care and support discharge planning.
- Educate patients and families on home programs, safety, and equipment use; ensure patient safety during therapy sessions.
Responsibilities also include possible coordination with two nearby facilities as needed to ensure continuity of care for residents.
Requirements
- SNF/rehabilitation experience preferred.
- Availability to work 40 hours per week on a flexible schedule (e.g., M-F with flexibility; Tue-Sat acceptable).
- Willingness to float between two nearby facilities within walking distance in the Napa area.
- Travel assignment experience is beneficial but not required.
