Responsibilities
- Provide direct, person-centered nursing care to residents per individualized plans of care in a long-term care setting.
- Administer prescribed medications and document administration accurately in accordance with policy.
- Monitor resident status, perform routine assessments, and report changes to the supervising nurse; escalate when needed.
- Assist residents with activities of daily living and mobility; support safe transfers and prevention of injuries.
- Collaborate with the interdisciplinary team (physicians, RNs, therapists) to update plans of care and ensure continuity of care; maintain thorough, accurate documentation.
- Uphold privacy, infection control, and safety standards; comply with regulatory requirements and facility policies.
Requirements
- Prior long-term care (LTC) and PCC experience
- Ability to work evenings and adapt to an 8-hour shift
- Availability for a 13-week travel assignment with guaranteed hours
- Demonstrated ability to function well within a healthcare team in a nursing home environment
