Responsibilities
- Administer medications and treatments according to care plans and physician orders, documenting responses and any changes.
- Monitor residents’ conditions, vital signs, and any acute changes; report findings to the supervising nurse promptly.
- Provide routine nursing care and support for activities of daily living, including mobility assistance and personal care.
- Perform wound care, dressing changes, and other standard nursing procedures as delegated.
- Collaborate with the care team to update care plans, document progress, and ensure accurate charting and compliance with facility policies.
- Support falls prevention, infection control practices, and resident safety during all care activities.
Requirements
- Active Licensed Practical Nurse license applicable to the job location
- LPN diploma from an approved program
- Experience in a long-term care or skilled nursing setting preferred
- Ability to work a 12-hour shift schedule (07:00–19:30)
- Local candidate for a LOCAL ONLY assignment
