Responsibilities
- Conduct nutrition assessments for hospice patients receiving home visits and in affiliated sites.
- Develop, implement, and monitor individualized nutrition care plans aligned with patient goals and interdisciplinary team input.
- Provide medical nutrition therapy and counseling to patients and families; address nutrition-related symptoms and care goals.
- Document assessments, care plans, and progress in the patient record; ensure compliance with regulatory and professional standards.
- Collaborate with physicians, nurses, social workers, and other team members; participate in care conferences and discharge planning when appropriate.
- Educate patients and caregivers on nutrition management, hydration, texture modifications, and hydration strategies as part of end-of-life care.
Requirements
- Licensure: California Registered Dietitian license; if eligibility for the exam began after January 1, 2024, Master’s degree requirements apply per program eligibility rules, and licensure must be obtained within 6 months of residency completion.
- Education: Bachelor’s degree in food and nutrition or equivalent; Master’s degree required only if eligibility for examination began on/after 01/01/2024.
- Dietetic internship or its equivalent: Required.
- Certifications: Registered Dietitian credential; BLS (AHA or ARC) certification required.
- Experience: Clinical Dietitian experience preferred.
- Setting: Ability to work effectively in a home health/hospice environment and manage care in patients’ homes and community locations.
