Responsibilities
- Provide skilled physical therapy services within the legal scope of practice, including initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications per Medicare, state, and agency standards
- Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
- Deliver skilled interventions and maintenance therapy programs appropriate to the patient’s home-based environment
- Directly supervise PTAs and Home Health Aides in compliance with state practice acts
- Consult with physicians and interdisciplinary team members on treatment adjustments and patient progress
- Educate patients and caregivers on home exercise programs, mobility, and safety to promote independence
- Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges within agency timeframes
- Apply ICD-10 coding for accurate and compliant documentation
- Participate in in-service programs and quality initiatives; support continuous clinical excellence
- Maintain infection control, safety, and HIPAA compliance; document interventions timely
- Contribute to interdisciplinary meetings and ongoing education; adapt to changing responsibilities
- Travel to patient homes within a 45-mile radius of the Albuquerque area as required
Requirements
- Current Physical Therapist license in the state of practice (New Mexico)
- Doctor of Physical Therapy (DPT) or equivalent degree from CAPTE-accredited program (or as recognized by APTA/CAAHEA)
- CPR certification (current)
- Two years of clinical experience preferred; home health experience is a plus but not required
- Ability to exercise sound clinical judgment and work independently in varied patient settings
- Strong communication and interpersonal skills; proficiency with basic computer use and readiness to learn EMR/OASIS systems
- Reliable transportation with valid driver’s license and auto insurance for home visits
- Demonstrated compliance with Medicare/Medicaid Participation requirements and agency policies
