Responsibilities
- Conduct initial evaluations and ongoing assessments of residents’ physical therapy needs
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Administer therapeutic exercises, gait training, and functional activities; apply appropriate modalities
- Reassess progress, document outcomes in resident records, and communicate with an interdisciplinary care team
- Facilitate discharge planning and caregiver education; promote safety and independence
- Ensure compliance with safety, infection control, and documentation standards
Requirements
- Active Physical Therapist license in the United States or eligibility for Virginia licensure
- Availability to work 32 guaranteed hours per week, typically 5 days per week
- Ability to work in a long-term care/geriatric setting
- Strong communication and teamwork skills; effective collaboration with the care team
- Ability to document patient progress accurately and follow facility policies
