Responsibilities
- Provide direct nursing care to residents in the LTC unit, including monitoring vitals, assessing needs, and documenting changes.
- Administer medications per physician orders and care plans; ensure accurate MAR/documentation and compliance with facility protocols.
- Perform routine nursing tasks such as wound care, diabetic checks, ADL assistance, mobility support, and assistance with activities of daily living.
- Maintain patient safety, prevent falls, manage call bells, and respond promptly to changes in condition.
- Document care in the electronic medical record (PointClick Care) and communicate with the interdisciplinary team; participate in care planning and handoffs.
- Float within the same building as needed and adhere to shift schedule, dress code, and infection control practices.
Requirements
- Active Licensed Practical Nurse (LPN) license in New York; NURSYS verification of active licenses.
- Current Basic Life Support (BLS) certification.
- Education: Nursing diploma or associate degree from an accredited program.
- Experience: 1+ year of long-term care (LTC) experience; skilled in LTC resident care and chronic condition management.
- EMR experience, specifically PointClick Care, preferred.
- Ability to work in a LTC setting; capable of performing duties on an 8-hour evening shift; comfortable with resident mobility assistance and care coordination.
- Licensure and license verification, driver’s license with address matching demographics, and RTO (time off) must be included at submission.
- Local candidates within 75 miles may be considered at the local pay rate.
