Responsibilities
- Provide comprehensive nursing care to patients undergoing rehabilitation in a skilled nursing setting.
- Administer prescribed medications and monitor patient responses, reporting changes as needed.
- Perform and document ongoing assessments, track progress toward rehabilitation goals, and adjust plans as appropriate.
- Collaborate with physical, occupational, and speech therapy teams to implement and optimize the patient’s rehab plan.
- Monitor vitals, manage wounds, monitor for signs of complications, and coordinate with the care team for escalation as required.
- Ensure accurate documentation, participate in discharge planning, and educate patients and families about care plans and progress.
