Responsibilities
- Perform comprehensive occupational therapy evaluations for residents and establish individualized treatment plans targeting activities of daily living, upper-extremity function, mobility, and cognitive goals.
- Deliver therapeutic interventions and modalities to improve independence, safety, and quality of life; adjust plans based on progress and re-evaluations.
- Document assessment findings, treatment goals, progress notes, and discharge planning in accordance with regulatory and facility requirements.
- Collaborate with an interdisciplinary team (nursing, physical therapy, speech-language pathology, social work) to coordinate care and optimize outcomes.
- Educate residents and families on therapy goals, home exercise programs, adaptive equipment, and safety strategies; participate in care conferences as needed.
- Support smooth transitions of care and contribute to appropriate discharge planning and follow-up recommendations.

Requirements
- Education: Master’s degree in Occupational Therapy from an accredited program.
- Licensure: California OT license or eligibility for licensure; ability to obtain and maintain licensure as required.
- Experience: 1+ year of clinical OT experience in acute care or SNF settings preferred.
- Certifications: NBCOT certification preferred or standard eligibility guidelines; ability to maintain licensure requirements.
- Skills: Strong documentation, communication, and collaborative practice with an interdisciplinary team; comfort working in a PRN scheduling model and within a skilled nursing facility environment.
