- Deliver evidence-based treatment and document progress, adjustments, and outcomes in resident records.
- Collaborate with an interdisciplinary team to coordinate care, discharge planning, and adaptive strategies to enhance quality of life.
- Participate in care conferences and contribute to goal setting, progress notes, and administrative documentation.
Requirements
- Ability to work effectively in a skilled nursing facility setting
- Willingness to commit to a 13-week travel assignment
- Strong written and verbal communication skills; accurate charting and documentation
- Ability to follow facility policies, safety protocols, and patient confidentiality requirements
- Eligibility to work in the United States
