Registered Nurse – Case Manager in Everett, Washington. Travel assignment with Healthforce Healthcare LLC. Start date: 05/19/2026. Duration: 8 weeks. Weekly pay: $2,043–$2,122. Responsibilities
- Lead discharge planning and care coordination for patients within acute hospital care, ensuring safe transitions to the next level of care
- Collaborate with an interdisciplinary team, including Social Services, to determine appropriate disposition and resources
- Manage a caseload of approximately 25–30 patients, documenting assessments, plans, and referrals in EPIC
- Educate patients and families on post-discharge needs, follow-up care, and community resources
- Support cross-campus and cross-unit care coordination to optimize patient flow and bed utilization
Requirements
- Active RN license (Washington) or pending license accepted
- BLS certification
- Minimum 1 year of acute care experience
- Discharge planning / care coordination experience
- EPIC charting experience
- Ability to float across Med-Surg, LLOS, ED and to work across two campuses
- First-time travel nurses accepted
- Not accepting locals
- Pre-approved time off (RTO) within standard window; 3 of 5 major holidays required
- Orientation ~30 hours; onboarding modules ~16 hours; pre-start modules self-reported
