Responsibilities
- Administer prescribed medications and treatments per physician orders, ensuring accuracy and safety; document in the electronic health record.
- Monitor resident status with regular vital signs, assessments, and observation; report changes promptly to the supervising RN.
- Assist with activities of daily living, mobility, hygiene, nutrition support, and wound care; maintain resident comfort and safety.
- Perform routine procedures within scope and facility policy; support infection control and safety standards.
- Collaborate with the interdisciplinary team to develop and revise care plans; provide clear handoffs to the next shift.
- Maintain accurate documentation and ensure compliance with regulatory requirements and privacy standards.
Requirements
- Active Vermont Practical Nursing license
- Ability to work Evening and/or Overnight shifts
- Willingness to participate in a travel assignment
- Experience in a long-term care or skilled nursing setting is preferred
- Strong communication and teamwork skills
