Travel Licensed Practical Nurse Long-Term Care Job
Licensed Practical Nurse (LPN) – Long-Term Care travel assignment in Geneva, New York. Start date 07/07/2026 for a 14-week contract. Weekly pay range $640-$687 per week. No guaranteed hours. Shift: Part Time evenings, two 12-hour shifts, with every other weekend and holiday. Setting: skilled nursing facility providing long-term resident care. Responsibilities
- Provide direct and indirect skilled nursing care to residents on the assigned unit under the supervision of a registered nurse.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the Interdisciplinary Team.
- Contribute to creation and ongoing refinement of an age-appropriate, culturally relevant plan of care, involving residents, families, and the healthcare team.
- Recommend revisions to the plan of care based on evaluation of resident outcomes and coordinate referrals to ensure comprehensive care.
- Maintain current nursing knowledge and competency; participate in continuing education and inservice activities; assist with mentoring and orientation of staff; provide leadership under the direction of the RN.
- Deliver care aligned with geriatric needs and collaborate with physicians and ancillary staff to promote safety and well-being.
Requirements
- NY Licensed Practical Nurse licensure (NY LPN)
- BLS certification through American Heart Association or RQI program
- Minimum 1 year of experience as an LPN, with prior LTC facility exposure
- Ability to provide care to adult and geriatric residents and to work the stated shift schedule
- Must be able to begin full-day orientation
- Demonstrated ability to assess patient data, communicate clearly with the care team, and follow established policies and procedures
- Commitment to safety and infection control, including handling of exposure to bloodborne pathogens
