Travel Registered Nurse Case Manager Job
Registered Nurse – Case Manager, Everett, WA. Start date 06/15/2026 for 13 weeks. Weekly pay range $2,217–$2,366 per week with guaranteed hours of 72 hours per 2-week period. Shift: 5x8 Day shifts. Setting: hospital-based care coordination and discharge planning. This travel - temporary assignment requires cross-campus coverage and collaboration with an interdisciplinary team.

Responsibilities
- Lead discharge planning and care coordination across the acute inpatient workflow, focusing on transitions to the next level of care.
- Perform needs assessments, develop and implement individualized plans of care, and coordinate post-acute services and durable medical equipment as appropriate.
- Collaborate with physicians, nursing, Social Services, and the broader care team to optimize patient outcomes and ensure smooth transitions.
- Document care plans and progress in EPIC; maintain accurate, timely, and compliant charting and education for patients and families.
- Manage a diverse patient population (neonates through geriatrics) and support care across related units; may float between units and campus locations as needed.
- Support adherence to hospital and regulatory standards, including holiday coverage and orientation requirements.
Requirements
- Active RN license in Washington state (or pending licensure accepted at submittal)
- BLS certification required; EPIC charting experience preferred
- Minimum 1 year of acute care experience with discharge planning or care coordination
- Nursing degree (ASN/BSN/MSN) required
- Ability to work in a hospital-based Case Management environment; 5x8 day schedule; ability to float to related units and between campus locations; willingness to work designated holidays
- Non-local candidates only (local candidates not accepted); relocation may be required
- References: two references covering at least 1 year within the last 3 years
- Background/Nursys disclosure: disclose any issues at submittal
