Physical Therapist – Home Health travel assignment in Tuba City, Arizona. Start date 08/24/2026 for a 13-week engagement. Weekly pay ranges from $2,377 to $2,495 per week. No guaranteed hours. Shift: 5x8 days (Monday–Friday, 8:00 a.m.–5:00 p.m.). Setting: surgical center environment supporting a broad scope of PT services.

Responsibilities
- Provide quality physical therapy services to a predominantly Native American population, focusing on prevention and treatment of physical disability or disease
- Manage and implement the physical therapy program; perform evaluations and treatments for inpatients and outpatients, ages birth through geriatric
- Deliver specialty services as indicated (e.g., wound care, prosthetic/orthotic clinic, geriatric and functional clinics, high school clinic, spinal cord clinic, assistive technology/equipment evaluation, DME/PROS)
- Collaborate on development and direction of the PT section, including quality improvement, safety, and policy procedures
- Prepare for Joint Commission standards, participate in staff education, meetings, and in-service activities
- Support credentialing processes and maintain compliance with medical staff requirements
Requirements
- Bachelor’s degree from a CAPTE-accredited School of Physical Therapy
- Valid, current, full, unrestricted license to practice in any U.S. state, District of Columbia, or Puerto Rico during tenure
- Current Basic Life Support (BLS) certification from the American Heart Association
- Fingerprinting must be submitted and cleared prior to start
- Must meet credentialing requirements to serve on the medical staff
- Ability to work in both inpatient and outpatient clinical settings
