- Reassess residents regularly and adjust treatment plans to meet goals
- Collaborate with an interdisciplinary team and communicate with families/caregivers
- Support discharge planning and coordinate patient progress with ongoing therapy needs
Requirements
- Ability to provide occupational therapy services in a long-term care setting
- Willingness to travel and participate in a shared assignment across two facilities
- Strong documentation and communication skills, with adherence to regulatory and facility policies
- Availability to meet the start date and complete the 13-week assignment
