Responsibilities
- Administer medications and basic nursing care under supervision, following physician orders and up-to-date care plans
- Monitor residents’ health status, document vital signs, changes in condition, and care activities
- Perform routine nursing tasks and assist with activities of daily living as needed
- Maintain accurate charting and documentation in the patient record; report changes to the supervising nurse
- Collaborate with the registered nurse and interdisciplinary team to coordinate care and ensure patient safety
- Support resident safety, fall prevention, hygiene, and infection control practices
Requirements
- Active LPN license in West Virginia
- Graduation from an approved practical nursing program
- Experience in long-term care or geriatric nursing preferred
- Ability to work a rotating night-shift schedule and every-other weekend
- Willingness to travel for a temporary assignment
