Responsibilities
- Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Serves as co-leader in an interdisciplinary approach to patient groups.
- Deliver patient-centered physical therapy interventions in a home health setting, adjusting plans of care as patients progress.
- Document patient progress, modify goals, and contribute to care coordination with the interprofessional team.
- Educate patients and families on home exercise programs, safety with activities of daily living, and mobility strategies to support independence.
Requirements
- Availability for a Day shift schedule (8 hours) on a travel assignment with 40 hours per week.
- Willingness to work in a home health - hospice setting.
- Strong collaboration skills to work within an interdisciplinary team.
- Ability to document and communicate patient progress effectively and maintain appropriate care records.
- Adaptability to a travel assignment and to providing care in patients’ homes.
