Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives
- Serve as co-leader in an interdisciplinary approach to patient groups, collaborating with other care team members
- Perform patient evaluations, develop goals, implement plans of care, and document progress; adjust as needed
- Provide patient and caregiver education on exercises, safety, and home care strategies
- Ensure documentation compliance and adherence to agency policies and regulatory requirements
- Monitor patient progress, communicate changes to the care team, and participate in plan-of-care updates
Requirements
- Active physical therapy license (state-specific licensure required)
- Degree in physical therapy (DPT or equivalent)
- Ability to work in a home-based care environment and travel to patient homes
- Strong collaboration and communication skills for interdisciplinary teams
- Willingness to commit to a 13-week travel assignment in the designated location
