Responsibilities
- Conduct initial and ongoing PT assessments in patients’ homes and develop individualized treatment plans.
- Deliver therapeutic interventions focused on improving mobility, strength, balance, and functional independence.
- Provide gait training, therapeutic exercises, and manual therapy as appropriate.
- Educate patients and family/caregivers on exercise programs, safety, and home exercise implementation.
- Document patient progress, update plans of care, and communicate with the care team and supervising clinicians.
- Ensure treatment delivery complies with regulatory standards and safety guidelines for home health care.
Requirements
- Active physical therapy license in Pennsylvania (or eligibility to practice in the state)
- Doctor of Physical Therapy (DPT) degree or equivalent
- CPR/BLS certification required
- Prior clinical experience in home health or outpatient PT preferred
- Ability to work in patients’ homes and travel within the service area
- Strong communication and documentation skills
