Responsibilities
- Provide skilled nursing care to geriatric patients in an acute care setting, collaborating with physicians and the multidisciplinary team
- Lead and coordinate the care plan, monitor patient progress, and adjust interventions as the clinical picture evolves
- Serve as liaison among the patient, family, nursing staff, physicians, and other care team members to optimize outcomes and discharge planning
- Assess complex patients, including high ED utilizers, chronic pain patients, and those with substance use or housing challenges, ensuring appropriate resource use
- Facilitate transfers to appropriate levels of care and ensure continuity of information for post-discharge planning
- Support regulatory and accreditation activities by documenting care, outcomes, and quality indicators
Requirements
- Valid Registered Nurse license in the state of practice (California)
- Basic Life Support (BLS) certification required
- RN Case Management (CM) certification required
- Minimum of two years of recent experience in the field; care management or IP CM experience preferred
- Education: BSN preferred
- Ability to work in a fast-paced hospital environment and to manage a heavy workload
- Eligible to work in the United States without sponsorship
