Responsibilities
- Conduct comprehensive speech-language pathology evaluations and diagnoses for residents in the facility
- Develop, implement, and monitor individualized treatment plans addressing communication and swallowing disorders
- Provide direct therapy and document progress notes for residents across relevant therapies
- Reassess resident progress regularly and adjust treatment plans as needed
- Collaborate with the multidisciplinary care team to coordinate care, discharge planning, and goals
- Educate residents and families about therapy goals, home exercise programs, and safety considerations
Requirements
- This is a travel assignment requiring eligibility to work in the United States
- Availability to work a full-time schedule with guaranteed hours (8:00 AM–4:30 PM)
- Ability to operate in a skilled nursing facility environment
- Willingness to commit to a 13-week contract duration
- Basic proficiency with clinical documentation and regulatory compliance in a SNF setting
