Travel Occupational Therapist – Home Healthcare in San Ramon, California. Start May 4, 2026 for a 26-week assignment with guaranteed 36 hours per week. Weekly pay range: $2,222 to $2,408 per week. Day shift in a home health setting.

Responsibilities
- Conduct initial and ongoing occupational therapy assessments in patients’ homes; develop and implement individualized plans of care to address functional goals
- Deliver direct OT interventions to improve activities of daily living, fine motor skills, cognition, and mobility; provide training and education to patients and caregivers
- Perform home safety evaluations and recommend environmental modifications to support independence
- Complete necessary documentation, including OASIS-related activities (Re-Certification, RoC, Routine, Eval, Discharge, SoC, and related forms), and maintain accurate progress notes
- Collaborate with the interdisciplinary team and family caregivers to coordinate care, adjust treatment plans, and ensure continuity of services
- Travel to assigned patients within the service area; weekend coverage may be required to meet caseload expectations and patient needs
- Adhere to clinical policies, infection control procedures, and state/federal regulations; maintain licensure and required certifications
Requirements
- Current California Occupational Therapist license
- Basic Life Support (BLS) certification
- Valid driver’s license, automobile insurance, and vehicle registration
- Immunizations and health screenings: TB (annual), MMR, Varicella, Hepatitis B, Tetanus-Diphtheria-Acellular Pertussis (TDap within 10 years), annual influenza
- Clear drug screen and physical examination
- Ability to work in a home health setting and travel within the assigned territory
- Willingness to treat patients with Covid-19 infection or exposure per policy and facility requirements
