Responsibilities
- Deliver direct resident care in a LTC setting, including assessment, monitoring, and documentation of changes in condition.
- Administer prescribed medications and treatments; monitor for adverse reactions and report any concerns to the supervising nurse.
- Support care plans by collaborating with RNs, other LPNs, and interdisciplinary team members to ensure continuity and quality of care.
- Assist residents with activities of daily living (ADLs) and provide compassionate comfort, mobility support, nutrition, and personal hygiene with respect and dignity.
- Maintain high standards of safety and infection control; observe for safety hazards and implement appropriate precautions.
- Communicate clearly during shift handoffs, document accurately in the chosen electronic health record, and participate in quality improvement activities as needed.
Requirements
- Active LPN license in New Mexico (or eligible for licensure under state requirements)
- Current Basic Life Support (BLS) certification
- Graduate of an accredited practical nursing program
- Prior experience in long-term care or skilled nursing settings preferred
- Ability to work 3x12-hour daytime shifts and comply with local scheduling needs
- Local candidate or ability to work within the Albuquerque area
