Responsibilities
- Provide skilled physical therapy services to LTAC patients under the supervision of a licensed physical therapist.
- Implement and progress treatment plans, monitor patient response, and adjust as directed.
- Document patient assessments, treatment sessions, and progress in the medical record.
- Collaborate with an interdisciplinary team to support mobility, transfers, gait training, and functional goals.
- Support activities of daily living, device use, and safe wheelchair positioning; maintain equipment and perform safety checks.
- Adhere to infection control, safety standards, and facility policies; communicate patient status and treatment updates clearly.
Requirements
- Active Physical Therapy Assistant license in New Hampshire.
- Graduation from an accredited PTA program (associate degree required).
- CPR/BLS certification required.
- Ability to work in a skilled nursing facility LTAC environment.
- Strong communication and documentation skills; ability to collaborate with the care team.
