Responsibilities
- Deliver coordinated nursing care for an individual patient or a group of patients according to established standards and the nursing process.
- Assess, plan, implement, and evaluate patient care; analyze data to inform diagnoses and care decisions.
- Coordinate care with other disciplines, delegate appropriately, and supervise staff as assigned.
- Administer medications and treatments, monitor patient status, respond to changes, and promote a safe, healing environment.
- Provide patient and family education, participate in discharge planning, and support continuity of care.
- Document care in the electronic health record (EPIC) and communicate effectively with the care team to ensure seamless transitions.
Requirements
- Active Registered Nurse license in the state of practice (CA or state of assignment)
- Current Basic Life Support (BLS) certification; CPR/BLS from an approved vendor as required
- EPIC experience is REQUIRED
- At least two years of recent practice in the field; ER or acute care experience preferred
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Facility/policy requirements: Hospital Fire and Life Safety (HLFS) certification required; Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications required per policy; NIH Stroke Scale (NIHSS) required
- Ability to work in a home healthcare - hospice setting and to function effectively within an ER-focused scope as part of the care team
