Responsibilities
- Administer medications and treatments as prescribed, with documentation of responses and any adverse effects.
- Monitor residents’ conditions, perform vital signs, and escalate changes to the supervising nurse promptly.
- Provide direct patient care, including assistance with activities of daily living and comfort measures.
- Maintain a clean, safe environment and follow infection control, safety, and privacy protocols.
- Document care provided in the resident’s chart and ensure clear communication with the health care team.
- Collaborate with licensed nurses and others on the interdisciplinary team to implement and adjust care plans.
Requirements
- Current LPN license (active) appropriate for the assignment
- Graduation from an accredited practical nursing program
- CPR/BLS certification
- Ability to work in a long-term care setting and to perform duties associated with a 12-hour shift pattern
- Effective communication skills and teamwork orientation
- Prior experience in long-term care or a similar setting is a plus
