Responsibilities
- Provide direct nursing care for patients or groups in a home hospice setting, following established standards and the nursing process.
- Collect and analyze assessment data, develop and implement care plans, and evaluate patient progress; coordinate with the interdisciplinary team.
- Administer prescribed medications and treatments, monitor responses, manage urgent situations per policy, and document care in the electronic health record (EPIC).
- Educate patients and families, participate in discharge planning, and maintain a safe, compassionate environment.
- Supervise and direct the activities of assigned nursing staff during the shift; delegate duties as appropriate and ensure adherence to policies.
Requirements
- Active RN license in California (state of practice).
- Minimum 2 years of recent nursing experience; acute care facility experience preferred.
- BLS certification required; CPR certification acceptable as an alternative per policy.
- ACLS and PALS certifications required per policy.
- Hospital Fire and Life Safety (HLFS) certification required.
- NIH Stroke Scale certification required.
- EPIC experience required.
- BSN degree preferred.
- Ability to work in a home healthcare – hospice setting.
