Responsibilities
- Provide comprehensive case management for a defined patient population from pre-admission through post-discharge, collaborating with the multidisciplinary team to optimize outcomes.
- Act as a liaison among the patient, family, nurse, physicians, and other providers to coordinate and ensure smooth transitions of care.
- Develop, implement, revise, and report on the case management plan; monitor progress and adjust as needed to meet quality and patient-centric goals.
- Serve as a clinical resource to the care team on specific patient items, offering guidance on care plans, resources, and best practices.
- Coordinate the continuum of care, ensuring timely progression through hospitalization and subsequent steps in the care pathway.
- Support efficient patient and family flow through the planning and execution of care, discharge planning, and follow-up in the hospice/home health context.

Requirements
- Active Registered Nurse license in California
- Relevant case management experience, preferably in hospice or home health settings
- Education: RN degree (ADN or BSN) required; BSN preferred
- Strong communication, coordination, and advocacy skills
- Ability to work within a home-based hospice/home health care environment and collaborate with a multidisciplinary team
