Licensed Practical Nurse | Long-Term Care | Waterloo, New York, United States | Start 06/09/2026 | 14 weeks | $1,428-$1,453 per week | Guaranteed hours: none | Shift: 5x8 evenings | Setting: Skilled Nursing Facility, travel - temporary assignment. Responsibilities
- Collect data on residents’ physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team
- Contribute to an age-appropriate, culturally relevant care plan reflecting nursing diagnoses and interventions, involving the resident/family and healthcare team
- Recommend revisions to the plan of care after outcome evaluation and coordinate referrals as needed; collaborate within the nursing scope
- Provide direct and indirect skilled nursing care under RN supervision; participate in mentoring and orientation of staff; maintain current nursing knowledge through education
- Administer medications and treatments per policy, monitor patient responses, and document appropriately
- Maintain safety, infection prevention, and environmental safety; support geriatric residents and promote a safe care environment
Requirements
- Current, active NY Licensed Practical Nurse licensure
- BLS certification (American Heart Association or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to perform rotations on D/E/N/W schedules and participate in holiday coverage
- Demonstrated ability to provide care for adult and geriatric residents and work effectively with an interdisciplinary team
- Must be able to start orientation on a Tuesday or Thursday and fulfill the 5x8 evening shift schedule
