Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility setting, with duties aligned to home-health responsibilities as applicable
- Develop, implement, and revise plans of care based on initial assessment, ongoing reassessment, patient history, diagnoses, physician orders, and goals; include discharge planning when appropriate
- Administer and monitor treatments and modalities (e.g., heat, water, electrical stimulation) using equipment such as whirlpool, electrical stimulation, and ultrasound, following established guidelines
- Document all skilled care thoroughly, timely, and accurately to meet Medicare, state, federal, JCAHO, payer, and local policy requirements
- Communicate significant clinical findings and changes to the physician and interdisciplinary team; provide education to clients and caregivers to promote independence and safety
- Supervise or train Physical Therapy Assistants and Home Health Aides when applicable, and participate in staff meetings, case conferences, and quality improvement activities
- Maintain client confidentiality, participate in quality management activities, and contribute to a high level of customer service for internal and external customers

Requirements
- Active Physical Therapist license to practice in the United States
- Graduation from an accredited physical therapy program (professional degree appropriate to licensure)
- Experience delivering PT services in SNF and/or home-health settings preferred
- Ability to perform supervisory duties for PTAs and Home Health Aides as applicable, and to work within Federal/State guidelines and local policy
- Strong documentation, communication, and teamwork skills; knowledge of Medicare, State, Federal, and payer requirements is a plus
- Ability to work in a skilled nursing facility environment and to support patient rehabilitation goals
