Responsibilities
- Conduct initial evaluations and ongoing assessments of residents to establish an OT Plan of Care.
- Implement therapy interventions to improve activities of daily living, functional mobility, safety with transfers, and independence.
- Document progress notes, update goals, and contribute to discharge planning; participate in interdisciplinary team rounds.
- Recommend and educate residents and staff on assistive devices, adaptive equipment, and environmental modifications.
- Collaborate with nursing staff and therapy colleagues to coordinate care and ensure continuity of services.
- Maintain timely, compliant documentation and communicate changes in resident status to the care team.
Requirements
- Degree in Occupational Therapy from an ACOTE-accredited program.
- Active, unrestricted OT license or eligibility to practice in Texas.
- Experience in long-term care or rehabilitation settings preferred.
- Strong written and verbal communication skills with proficient documentation.
- Ability to work in a day shift and travel to the assignment location; commitment to a 13-week assignment.
