Responsibilities
- Conduct initial PT evaluations and develop individualized treatment plans to address mobility, strength, balance, and functional independence
- Deliver evidence-based physical therapy interventions and adjust plans as progress is made
- Reassess patients regularly, document progress, and update plans of care accordingly
- Collaborate with an interdisciplinary team including nursing, therapy assistants, and other care providers
- Educate patients and caregivers on exercises, safety, transfer techniques, and home exercise programs
- Maintain accurate records, ensure patient safety, and uphold infection control and standard precautions
Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent
- Active Physical Therapist license in Pennsylvania or eligible for licensure in PA
- 2+ years of clinical PT experience, preferably in geriatric rehabilitation or similar setting
- Ability to work in a travel assignment and adapt quickly to a new care environment
- Strong documentation, communication, and collaboration skills
- Must be able to commit to a 13-week assignment with the listed shift and hours
