Responsibilities
- Collect data on residents' physical and psychosocial condition and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of resident care reflecting nursing diagnoses and interventions, involving the resident, family, and healthcare team.
- Recommend revisions to the plan of care after evaluating resident outcomes; identify referrals to ensure all aspects of care are addressed.
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse; follow facility policies and procedures.
- Maintain current knowledge and competency through continuing education and inservice programs; participate in mentoring or orientation of staff; provide leadership within the professional practice setting.
- Maintain safety and infection control practices; handle tasks with potential exposure to blood, body fluids, or tissues in accordance with policy.

Requirements
- Licensed Practical Nurse (LPN) license, active in New York state.
- BLS certification (AHA or RQI required).
- Minimum 1 year of LPN experience, preferably in a long-term care setting.
- Ability to provide care to adult and geriatric residents and support developmentally appropriate care plans.
- Demonstrated ability to work under the supervision of a Registered Nurse within a skilled nursing environment.
