Physical Therapist – Home Health in Clearlake, California. This travel, temporary-assignment opportunity is for a DPT with home health experience and will operate in a critical access hospital setting. Start date is June 22, 2026, for a 13-week contract. Work a Monday–Friday day shift (5x8), 7:00 a.m. to 3:30 p.m., with no weekend requirements. Weekly pay ranges from $2,065 to $2,128. Travel mileage is reimbursed at the government rate.

Responsibilities
- Conduct comprehensive PT evaluations for home health patients and develop an evidence-based Plan of Care aligned with HCHB and OASIS requirements
- Deliver individualized therapy focused on gait, balance, strength, mobility, pain management, and functional rehabilitation
- Provide patient and caregiver education, training, and home program instruction to promote independence and safety
- Document assessment findings, treatment plans, progress notes, and OASIS data accurately in the medical record; communicate changes with the interdisciplinary team
- Ensure patient safety during visits, maintain infection control standards, and adhere to AHA BLS requirements
- Coordinate with care teams to optimize discharge planning and transitions of care
Requirements
- Doctor of Physical Therapy (DPT) degree
- California Physical Therapist license (or eligible for licensure)
- AHA BLS certification
- Minimum 1 year of home health experience
- Proficiency with HCHB and OASIS documentation
- Ability to perform home health PT visits, travel between patient locations, and work the listed Monday–Friday daytime schedule
