Travel Registered Nurse Case Manager Job
Registered Nurse – Case Manager in Everett, Washington. Travel - temporary assignment starting May 4, 2026 for Weeks. Weekly pay: $2,183–$2,321 per week. Guaranteed hours: 72 hours per 2-week period. Shift: 4x10 Days. Setting: hospital with Level II Trauma Center and 571 staffed beds across two nearby campuses.

Responsibilities
- Lead discharge planning and care coordination for patients across Med-Surg, Long LOS, and ED; develop and implement patient-specific plans of care; coordinate with Social Services and post-acute providers; arrange DME as needed.
- Document and track patient progress in EPIC; participate in interdisciplinary rounds with the care team to ensure timely transitions to the next level of care.
- Conduct needs assessments, identify barriers to discharge, and arrange appropriate post-acute services to support safe, timely discharge.
- Collaborate with physicians, nurses, social work, and other disciplines to optimize patient outcomes and maintain CMS regulatory compliance.
- Manage a patient load with an approximate ratio of 25–30 patients and ensure continuity of care across unit assignments; participate in schedule that allows block planning and cross-unit float as needed.
Requirements
- Active Registered Nurse license in Washington (pending license acceptable for submission)
- BLS certification
- Minimum 1 year of acute care experience
- Discharge planning / care coordination experience
- EPIC charting experience required
- Ability to work in a hospital setting and float across units; block scheduling; weekend and holiday commitments as outlined
