Responsibilities
- Deliver direct, person-centered nursing care to residents within the LPN scope of practice under supervision or physician guidance.
- Monitor residents’ conditions, perform routine assessments, and report changes promptly to the supervising nurse.
- Administer prescribed medications and perform routine treatments per plan of care, documenting accurately.
- Support activities of daily living, nutrition, hydration, and mobility to promote resident well-being.
- Collaborate with the interdisciplinary team to update and implement care plans; participate in care conferences as needed.
- Maintain infection control practices, safety standards, and thorough charting in electronic health records or logbooks.
Requirements
- Active Licensed Practical Nurse license (NM) or eligibility to practice in New Mexico.
- Current CPR/BLS certification.
- Graduate of an accredited practical nursing program.
- Relevant experience in long-term care or skilled nursing settings preferred.
- Ability to work a 3x12-hour day shift schedule and adapt to a fast-paced care environment.
- Strong communication, teamwork, and documentation skills.
