Licensed Practical Nurse (LPN) travel assignment in a Skilled Nursing Facility unit, located in Utica, New York. Start date: 11/02/2026. Duration: 13 weeks. Responsibilities
- Provide skilled nursing care to residents on the LTC unit under the supervision of an RN, following the plan of care
- Administer medications and treatments per physician orders; document care in the EMR (PointClickCare)
- Monitor residents’ condition, identify changes, and communicate with the nursing team and interdisciplinary colleagues
- Assist residents with activities of daily living and support memory care and dementia residents
- Participate in discharge planning, admissions within the unit, and accurate record-keeping
- Adhere to safety, infection control, and privacy standards; participate in quality improvement and care coordination

Requirements
- Active LPN license in New York; Nursys verification required; NY license must be current
- Current BLS certification
- 1+ year of LTC facility experience
- EMR experience with PointClickCare
- Ability to work on a 2:00 pm–10:00 pm shift and float within the same building
- Driver’s license with address that matches demographics
- RTO (time off) must be included at time of submission
- Local rate eligibility for applicants located within 75 miles
