Licensed Practical Nurse (LPN) – Long-Term Care | Waterloo, NY | Start 04/28/2026 | 13 weeks | $1,381–$1,448 per week | 0 guaranteed hours | Evening shift, 5x8 | Travel temporary assignment in a skilled nursing setting.

Responsibilities
- Provide direct and indirect skilled nursing care to adult and geriatric residents under the supervision of an RN.
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to a developmentally appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions to achieve outcomes, involving the resident/family and healthcare team.
- Revise the plan of care after evaluating resident outcomes and coordinate referrals as needed; collaborate with residents, families, and team to ensure comprehensive care.
- Maintain current knowledge and competency through continuing education and in-service programs; mentor or orient newer staff as directed.
- Provide leadership in the professional practice setting under the direction of the RN, and participate in shift rotations (D/E/N/W) as assigned, while adhering to safety and infection control practices.
Requirements
- Active New York LPN licensure
- Basic Life Support (BLS) certification (AHA or RQI)
- Minimum 1 year of LPN experience; LTC experience preferred
- Experience caring for adult and geriatric residents
- Ability to work evenings and participate in D/E/N/W rotations as assigned
- Completion of an approved LPN program; ability to practice within a skilled nursing setting
