Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized plans of care for patients in the hospital-based setting.
- Administer therapeutic interventions to improve mobility, strength, balance, gait, and activities of daily living.
- Monitor progress, document outcomes, and adjust treatment plans in accordance with policy, payer guidelines, and patient goals.
- Collaborate with physicians, nurses, and the rehabilitation team to coordinate care, discharge planning, and goal attainment.
- Educate patients and families on exercises, safety, and fall prevention; provide clear home exercise guidance as appropriate.
- Maintain compliance with HIPAA, OSHA, and infection control standards; ensure accurate charting and timely communications.
Requirements
- Active Physical Therapy license in the state of practice (California)
- CPR or Basic Life Support (BLS) certification from an approved vendor
- Minimum of 2 years of relevant clinical experience
- Ability to work in a hospital-based, day-shift environment
- No on-call requirement as part of the scheduled shift
