Registered Nurse – Home Health, travel assignment in Cottonwood, Arizona. Start date May 11, 2026. 13-week contract with 40 guaranteed hours per week. Shift: 8-hour days. Weekly pay range: $2,117–$2,141. Setting: Skilled Nursing Facility.

Responsibilities
- Administer skilled nursing care for home health/hospice patients in stable and emergent situations; coordinate care with the interdisciplinary team, patient/family, and referring agency; develop and evaluate the Plan of Care (POC).
- Assess patients and families to identify physical, psychosocial, and environmental needs; document findings and participate in case conferences and team meetings.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care; maintain up-to-date patient records reflecting visits, problems, plans, actions, and goals.
- Provide care with infection control measures; educate patients and families on care, medications, home safety, and related topics; promote self-care practices.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service.
- Assess and reassess pain; apply appropriate pain management techniques; coordinate referrals to appropriate services and monitor third-party payer requirements; participate in on-call duties as assigned.
Requirements
- Current state RN license (Arizona) – Required
- 1–2 years RN clinical care setting experience; 2+ years Home Health experience – Required
- Homecare Homebase software experience – Required
- OASIS experience – Required
- CPR certification – Required; CHPN certification – Preferred
- Valid driver's license; reliable transportation and insurance – Required
- Education: Associate Degree in Nursing or Nursing Diploma – Required; Bachelor of Science in Nursing – Preferred
