Licensed Practical Nurse — Long-Term Care in Saint Albans, Vermont, United States. Responsibilities
- Administer medications and treatments per physician orders and facility policy
- Monitor residents for changes in condition and document findings in the chart
- Provide direct resident care, including activities of daily living and comfort measures
- Collaborate with the registered nurse, physicians, and the interdisciplinary team to update care plans
- Adhere to safety protocols, infection control practices, and incident reporting
- Assist with care coordination, including family communication and discharge planning as needed
Requirements
- Active LPN license (state-specific) in good standing or eligibility for Vermont licensure
- Ability to work evening and/or overnight shifts
- Prior experience in long-term care or skilled nursing facility preferred
- Strong communication and clinical documentation skills
- Graduation from an approved Licensed Practical Nurse program
