Role Specifics and Benefits:
- Key responsibilities: As a Home Health RN, you’ll conduct thorough in-home assessments, develop and adjust individualized care plans, manage complex medication regimens, perform wound care and IV therapy as needed, monitor vitals and symptoms, and coordinate with physicians, therapists, and social services to optimize outcomes. You’ll educate patients and families on disease management, nutrition, hygiene, safety, fall prevention, and equipment use. You’ll implement and evaluate palliative and end-of-life comfort measures when appropriate, coordinate transitions of care, and document meticulously to ensure continuity and compliance. Telehealth check-ins and timely communication with the broader care team will support proactive care, while you remain responsive to urgent needs through clear escalation pathways.
