Licensed Practical Nurse – Long-Term Care – Claremont, New Hampshire, United States. Travel assignment starting 04/28/2026 for 13 weeks, with a weekly pay range of $1,584 to $1,647 and guaranteed 40 hours per week. Shift: Nights, 11:00 p.m. to 7:00 a.m., 5x8. Setting: LTC – Nursing home. This is a temporary travel opportunity through Healthforce Healthcare LLC.

Responsibilities
- Provide direct patient care in accordance with physician orders; receive, transcribe, and carry out orders; document MARs/TARs and electronic health records.
- Administer medications and treatments as ordered; supervise LNA staff within scope of practice; ensure accurate documentation and medication control.
- Participate in physician rounds on assigned units; communicate changes in resident status and prepare detailed shift reports for oncoming staff; assist with 60-day reviews as needed.
- Administer oxygen therapy and enteral feedings per policy; perform wound care and manage wound vacs; monitor infection control practices on the unit.
- Manage admissions, discharges, and transfers; complete 24-hour unit reports; coordinate with the Nurse Manager/Nursing Supervisor regarding resident issues and staff assignments.
- Support orientation and ongoing education for nursing staff; reinforce chain of command and participate in in-service trainings as scheduled.
Requirements
- Active New Hampshire Licensed Practical Nurse license
- BLS (AHA) certification; IV certification or willingness to obtain during onboarding
- 3+ years of current nursing experience within the last 6 months
- Must be willing to float to units with experienced staff
- Covid-19 vaccination required
- Own transportation required due to rural location and housing options
